Location: Dane County
Hybrid Position

Schedule: Part-time (22.5 hours/week) or Full-time (37.5 hours/week)
Flexible Schedule

Salary: $68,000-$75,000

Position Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) to join our team. The RN will provide case management services and oversee RN-delegated tasks to support caregivers in the community. This role offers the opportunity to coach, mentor, and lead individuals in their professional development while ensuring high-quality care standards.

Key Responsibilities:

  • Provide case management services to caregivers and clients in the community
  • Train on personal cares and delegated nursing tasks to caregivers, ensuring compliance with state regulations
  • Conduct assessments and develop care plans that align with clients' needs
  • Offer coaching and mentorship to caregivers, fostering skill development and confidence
  • Collaborate with interdisciplinary teams to enhance care coordination
  • Maintain accurate documentation and uphold best practices in community nursing


Minimum Education/Experience and Requirements:
Required:

  • Must be 18 years old or older
  • Must be authorized to work in the United States
  • Associate's degree in nursing
  • Current licensure as a Registered Nurse under s. 441.06, Wisconsin Statues
  • Valid driver's license, reliable transportation, and current auto insurance to travel throughout Dane County
  • Must be able to push, pull, and/or lift up to 35 pounds
  • Work around environmental hazards such as odors, second-hand smoke, animals, cleaning supplies, adverse living conditions, etc.
  • Satisfactory Results of a WI Caregiver Background Check


Preferred:

  • Bachelor's in nursing
  • 1-year supervisory experience in personal care services, community-based home care (non-skilled nursing) or related program
  • 3 years nursing experience in personal care services, or related program
  • Experience working with people with disabilities and elderly
